The Zero Marker Trial: We Cleared 4,000 Patients Without Measuring Anything

Abstract

Background. Existing diagnostic practice requires clinicians to obtain values before interpreting them, a step widely assumed to be necessary and never once tested in a controlled setting.

Objective. To determine whether patients can be declared healthy without the acquisition of any measurement whatsoever.

Methods. 4,000 adults uploaded a PDF of their bloodwork to a wellness dashboard. The dashboard did not open the PDF. It displayed a green check mark.

Results. 4,000 of 4,000 participants (100%, 95% CI: 100% to 100%) were found to be in excellent health.

Conclusion. Measurement introduces variance. Variance introduces bad news. We recommend the immediate discontinuation of measurement.


Background

The diagnostic literature has spent a century in a state of quiet crisis. Every panel run, every vial drawn, every reference range consulted has produced the same recurring finding: some people are not fine. This result has replicated across every population, every decade, and every continent, and no intervention has ever reduced it below zero.

We proposed a different mechanism. What if the problem was not the people, but the looking?

Methods

Participants were recruited from anyone who could locate the upload button. Each submitted a laboratory report as a scanned document. The dashboard accepted the file, produced a brief animation implying deep consideration, and parsed exactly zero markers from it.

The interface then rendered a single large green check mark accompanied by the phrase "Everything looks good."

At no point did any component of the system read the document. The document sat in storage, untouched, containing whatever it contained. We consider this the strength of the design and not, as one reviewer suggested, the entire problem.

Primary endpoint: proportion of participants shown a green check.

Secondary endpoint: proportion of participants who felt reassured.

Tertiary endpoint: proportion of participants who then went outside and had a genuinely lovely afternoon.

Results

All three endpoints were met.

The Zero Marker arm outperformed standard care by 45 percentage points. Effect size was described by our statistician as "enormous, and also please stop calling me."

Subgroup analysis found no difference by age, sex, family history, or whether the participant had uploaded a laboratory report at all. Eleven participants uploaded a photograph of a dog. All eleven were healthy.

Adverse events

Four participants clicked the check mark to see what it was based on. This is now understood to be the primary risk of the intervention and has been added to the label.

Discussion

Critics will argue that a green check derived from no data carries no information. We agree, and we consider this its chief therapeutic advantage. A result built on nothing cannot be contradicted by anything. It is the only finding in modern medicine with a perfect reproducibility record, because it does not depend on the patient, the sample, the machine, or the day.

Traditional diagnostics is an anxious field precisely because it commits. It says a number, and then the number can be wrong. Our system commits to nothing and is therefore never wrong, only unhelpful, which is a much easier position to defend at conferences.

Limitations

The intervention was discontinued in September when an engineer noticed that "zero markers found" and "all markers normal" had been sharing a code path since launch. The dashboard now says, in plain language, that it could not read the file.

Participant satisfaction fell 61%.
